Diseases of the cardiovascular system are leading among the causes of premature death in our country (as in most countries of the world). The intense rhythm of life, chronic stress, alcohol and nicotine addiction, and inactivity lead to adverse and then dangerous changes in the blood vessels and heart of a person. Prevention, early diagnosis and treatment of cardiovascular diseases is a priority of modern healthcare. At the same time, the modernization of medical institutions is one of the most important tasks. After all, the art of doctors largely depends on the equipment that they can use in their work. Thanks to national project “Health” it became possible to carry out the most complex and unique operations on the heart and blood vessels in many regions of our country.

High-tech life-saving surgeries

At the Research Institute of Complex Problems of Cardiovascular Diseases of the Ministry of Health of Kuzbass, modernization has made it possible to introduce high-tech operations for transcatheter implantation of the aortic heart valve into clinical practice. Already in the first year of the development of the technique (in 2020), 6 patients underwent prosthetic heart valve replacement or replacement. Then the score is kept for dozens of lives.

By equipping with new equipment within the framework of national project “Health” doctors were able to apply technology that is as safe as possible for the patient. Therefore, after the operation, rehabilitation is not required. Within an hour and a half, a heart valve is changed for a person, while he is conscious, since the entire intervention is performed under local anesthesia.

– I am not writing on emotions now – I have so much gratitude and a keen sense of the line between this world of a normal, full life – as far as possible at the age of 69 – and what could happen if it were not for cardiac surgeons, – describes the rescued by doctors Anatoly Nikolaevich from the city of Leninsk-Kuznetsky. – I express my heartfelt gratitude and low bow to all medical staff – from doctors to nurses and technical staff – for highly qualified assistance, composure and well-organized work, careful and respectful attitude towards patients.

With new equipment, congenital pathology is not a sentence

In July 2022, cardiac surgeons of the Samara Regional Clinical Cardiological Dispensary named after A.I. VP Polyakova performed a complex operation on a two-year-old patient with a rare heart defect – Ebstein’s anomaly. The location of the tricuspid valve of the heart and the anatomy of the valves are disturbed in a child. The cavity of the right atrium is much larger than the norm, and the right ventricle is less than the norm.

This is one of the most difficult diagnoses in pediatric cardiovascular surgery. To perform the operation, both the highest skill of doctors and the most modern medical equipment are required. The hospital received it as part of the implementation national project “Health”.

– Such a congenital pathology (Ebstein’s anomaly) poses a threat to the life of the child. We performed a defect correction: we changed the anatomy and location of the tricuspid valve, – told Head of the Department of Cardiac Surgery and Cardiorheumatology Sergey Shorokhov.

Deep Freeze for Painless Recovery

When high-tech equipment is at hand, it gives doctors the opportunity to be creative in saving lives. So, in the Volga branch of the National Research Center for Transplantology and Artificial Organs named after Academician V. I. Shumakov of the Ministry of Health of Russia, extreme cold, or the method of cryoablation, was used to combat arrhythmia.

This is a modern and promising direction in cardiology for the treatment of cardiac arrhythmias. A special balloon-catheter was brought to the focus of arrhythmia, which, at a temperature of -50 ° C, freezes the cells pointwise. They lose the ability to conduct pathological electrical impulses. Importantly, this treatment allows for quick and less painful rehabilitation than with other methods of surgery.

IMPORTANT

Thanks to rehabilitation, the effectiveness of the treatment of patients with cardiovascular diseases increases significantly. In Russia, a special federal project “Optimal medical rehabilitation for restoring health” is being implemented. This is one of 42 initiatives for the socio-economic development of the Russian Federation until 2030, initiated by the government. The implementation of the project should increase the coverage of modern rehabilitation assistance.

dangerous carelessness

The fight against cardiovascular diseases is not only unique high-tech operations, but also daily routine work. In 2021, about 600,000 high-risk patients who are under dispensary observation received subsidized medicines. In 2022, as part of national project “Health” outpatient medicines, at home, are received by people who have had a myocardial infarction, coronary artery bypass grafting, angioplasty of the coronary arteries with stenting.

“It is necessary to take the drugs recommended by the doctor for the heart, even if the condition improves. If the adherence to treatment is low, if the patient cancels his medications on his own because “why poison the body” or “I feel fine anyway”, the heart will not thank for this”, – warns Evgeny Shlyakhto, Chief Freelance Cardiologist of the Russian Ministry of Health. According to statistics, upon admission to a cardiological hospital, only 15% of patients take drugs prescribed by a doctor, and 69% at discharge. Behind ignoring medication are repeated myocardial infarctions, early disability, progression of heart failure.

From telemedicine consultations to operations through micro-accesses in the skin

What other advanced approaches are used in the fight against cardiovascular diseases in our country:

• In 2022, it is planned to re-equip/re-equip 366 regional vascular centers and primary vascular departments.

• It is planned to purchase 5022 units of medical equipment, of which 139 units are “heavy machines”, including magnetic resonance and computed tomographs.

• The number of X-ray endovascular interventions, operations on vessels, which are performed through micro-accesses in the skin and with the help of special conductors – probes, under X-ray control, increased by 26% and amounted to about 252,000 in 2021. This means minimal skin tears, an accurate view areas of pathology and a quick return to active life after surgery. This development of high-tech surgery became possible primarily due to the renewal and development of the infrastructure of medical organizations for national project “Health”.

• In 2021, the National Medical Research Centers (NMRC) conducted more than 11,000 telemedicine consultations in the fields of “cardiology” and “cardiovascular surgery” for especially complex and difficult patients throughout the country.
